Performance Improvement Plan Templates: How to Choose and Use One

Choose a dedicated performance improvement plan (PIP) document for one employee’s objective, timeline, support actions and review dates. Keep the observations that inform the plan, evidence citations and review meetings in separate, traceable records. A template standardizes the process, but it does not establish that an assessment is accurate, consistent or legally sufficient.

For example, if customer responses fall below a documented service standard, the plan can state the target, measurement period, coaching and check-ins. Each reported result should link to its source record, while every review meeting should have its own dated record. This keeps the plan useful without treating a manager’s summary as the evidence itself.

A PIP is a controlled record of an improvement process, not a substitute for the evidence and human review that make the process fair and usable.

How to choose and use a performance improvement plan template

A performance improvement plan template is a repeatable structure for recording an improvement objective, expected standard, measurement period, support, owners and follow-up. Identify the process before selecting the document:

  • Formal employee-specific improvement: use a dedicated PIP document.
  • Proactive skill growth: use a development plan when there is no formal performance deficiency.
  • Recurring process failure: use corrective-action analysis to examine causes and prevention.
  • Project delivery reporting: use a project-level report rather than an employee PIP.

A PIP is not interchangeable with a routine performance review, development plan, project report or disciplinary decision. It organizes a process. It does not prove that expectations were communicated or that the underlying facts are sound.

Choose the right template type for the job

Tool Useful for Not a substitute for
Dedicated PIP document One employee’s formal goals, timeline and support plan. ClickUp describes its PIP template as a ready-to-use document for goals, timelines and support resources. Evidence records, HR review or an employment consequence.
Corrective-action Whiteboard Process gaps, root causes and preventive actions. ClickUp describes its Corrective Action Plan as a Whiteboard for this type of analysis. An employee-specific improvement plan.
Employee development plan Proactive growth and skill development. The current ClickUp page advertises 11 custom fields and five views, with examples including review date, assessment, timeline, staffing requirements and job position. A formal response to a documented performance deficiency.
Adjacent planning tool Reviews, onboarding, career planning, evaluation intake or project reporting, depending on the task. A dedicated PIP unless the employer adapts and governs it for that purpose.

Template contents and workspace availability can vary. Template.net currently lists editable PIP designs in several formats on its performance improvement category page. Check the specific file’s format, licensing and download requirements rather than assuming that an older URL still provides the same Word document.

Decision point

A corrective-action Whiteboard can diagnose a broken handoff, unclear procedure or other process failure. If the cause belongs to a process owner, assign the process fix there instead of converting the Whiteboard into a personnel plan.

What an actionable PIP template should capture

Choose or configure a template with enough structure to support a specific conversation and later review. Useful fields include:

  • Employee, manager and decision-owner identifiers.
  • Plan start and end dates, review dates and the observation period.
  • A factual issue statement, expected standard, measurable target and assessment method.
  • Actual performance values and links to source evidence.
  • Support actions, owners and due dates, such as coaching or relevant training.
  • Employee comments or acknowledgment and the process for determining next steps.

Separate facts from conclusions. Instead of writing “poor performance,” record the metric, period, expected value, actual value and source. A hypothetical plan might set a documented response-time target for a defined six-week period. That example illustrates structure, not a universal service benchmark.

Keep each record at the correct grain. One plan record represents one employee’s plan episode. One observation represents one metric, period and source event. One evidence record represents one citation. One review record represents one scheduled meeting. A project’s budget or delivery measure is not automatically evidence about one person. Review role, workload, dependencies and attribution first.

Proposed schema, not a ClickUp-published template: this example represents one raw observation. Store each supporting citation separately when several documents support the result.

{
  "employee_id": "E-1042",
  "plan_id": "P-2026-018",
  "metric_id": "response_sla",
  "period_start": "2026-10-01",
  "period_end": "2026-10-31",
  "expected_value": "95% within SLA",
  "actual_value": "91% within SLA",
  "source_event_id": "event-78421",
  "source_reference": "internal-record-reference-3"
}

For concurrent workers, the observation identity should include the source event and measurement period, not only the employee and date. Use a database-enforced uniqueness constraint or transactional upsert. A lookup followed by a create is not race-safe because two workers can both find no existing record.

employee_id + plan_id + metric_id + period_start + period_end + source_event_id

That is a proposed key for raw observations. A review meeting needs a different key, such as plan_id + review_sequence_number. A citation can use observation_id + source_document_id + source_locator. Reported summaries should use their own aggregation period and version, rather than reusing a raw observation ID.

Build the process before automating follow-up

Start with human review, not an automatic status change. Keep the observation, plan, support action and review meeting distinct. ClickUp Forms can collect submissions into a selected List, while Automations can respond to documented triggers with actions such as comments, messages or webhooks. Neither intake nor a reminder should decide whether an employee enters or completes a PIP.

01Record the observationThe manager records the event, expected standard, measurement period and source reference. The output is a dated raw observation.
02Review the issueHR or the designated reviewer checks the standard, evidence, role, resources, training and possible protected-process concerns. The gate is proceed, clarify, redirect or pause.
03Establish the plan and supportThe manager and employee document measures, dates, support owners and employee input in the dedicated plan.
04Prompt the scheduled reviewA date-driven automation notifies the assigned owner. Test the automation creator’s timezone, overlapping triggers and repeat behavior before launch.
05Record the meetingThe manager creates a separate meeting record with date, attendees, notes reference and progress against the stated criteria.
06Review closureThe designated decision owner reviews the complete record and applicable policy before documenting the next step.

ClickUp’s documented automation behavior includes date conditions based on the automation creator’s timezone, and multiple automations can share a trigger. Actions can consume monthly automation capacity, including when an action fails. Test schedules and duplicate behavior. A fired reminder is not evidence that a meeting occurred.

For an operationally configured ClickUp workflow, ClickUp setup and automation is an optional resource. It does not replace an employer’s HR review.

Practical workflow patterns for a template-based PIP

The following patterns distinguish documented platform capabilities from proposed field designs. Use deterministic rules for required fields, numeric thresholds, dates and duplicate detection. AI is optional and should be limited to drafting a summary of supplied records.

Trigger and input AI job Validation and action Fallback
Manager creates a plan: documented gap, standard, observation period and source references. None required. Check one plan episode, owner, dates, measurable objective, support and linked evidence before issuing the plan. If the standard or period is missing, return the draft for clarification rather than starting the plan clock.
Form-to-List intake: approved submitter sends employee ID, event date, factual observation and source reference. None required. A rule may route the item by category. Validate required fields and duplicates, then send the List item to a human reviewer. Redirect or request details when evidence is missing or the issue may concern a protected process.
Evidence-summary request: reviewer supplies linked observations and asks for a source-bounded summary. Summarize only supplied records and identify missing references. Do not assess failure or recommend discipline. Compare every material statement with the source and require named human approval before consequential write-back. Reject unsupported claims or incomplete citations and return the item to the source-record owner.
Review-date reminder: plan date arrives or approaches. None required. A deterministic reminder is sufficient. Send a comment, message or webhook, then require a separate human meeting record. Reschedule missed meetings and record the exception. Do not mark completion from the trigger alone.

A bounded AI draft could use the following proposed output. The schema check and source comparison happen before a human reviewer approves it.

{
  "plan_id": "P-2026-018",
  "summary_draft": "Draft summary based only on the supplied October observation.",
  "source_references": [
    "internal-record-reference-3"
  ],
  "missing_information": [],
  "review_required": true
}

ClickUp documents Autopilot Agent configuration concepts, but not a PIP-specific decision agent. Keep thresholds, deadlines, required fields and duplicate checks deterministic. For API-based task creation, the Create Task documentation states that required custom fields are ignored by default unless the relevant option is enabled. The generic Update Task endpoint does not update custom fields, so use the documented custom-field operation. Confirm that fields apply to the selected task type.

When to pause and review before issuing a PIP

Before treating a concern as an employee performance deficiency, check whether expectations were communicated, the employee had relevant tools and training, duties changed, and the measure is within the employee’s control. Training, coaching or resource changes may be support actions within a plan, but their presence does not decide whether a PIP is appropriate.

Route potential accommodation, protected leave, discrimination, harassment, protected complaint or retaliation concerns to HR before an employment action is determined. EEOC guidance supports attention to factual basis, consistent treatment and documentation in consequential employment actions. It is not a universal PIP standard.

Pause before issuing
  • Is the expected standard documented and were expectations communicated?
  • Does each material observation link to a source and defined period?
  • Have role changes, workload, tools, training and support needs been considered?
  • Does a protected-process concern require HR review first?

If the standard or evidence is missing, clarify it. If the issue points to resources or changed duties, address and document those factors. If a protected-process concern may be involved, pause for HR review. Otherwise, follow the employer’s established performance process. Any consequence should reflect policy and receive appropriate HR or counsel review rather than being prescribed automatically by a template.

Privacy, permissions and recordkeeping

Collect only information needed for the performance process. Do not put medical details or unrelated protected information in general-purpose PIP fields. Design access around need to know, then test visibility and editing with representative manager, employee, guest, form and administrator roles. ClickUp documents private custom fields, but actual behavior depends on permissions and workspace configuration.

Identify the employer’s authoritative personnel-record system and retention rules before using a work-management workspace as the only record. ClickUp documents Audit Logs as an Enterprise feature with 60 days of historical data. That is not a permanent employment-record audit trail. Decide what evidence to link versus copy, and confirm retention and export expectations with HR. A ClickUp audit may help review workspace configuration, but it does not establish PIP compliance.

Frequently asked questions about PIP templates

How long should a PIP last?

The sources reviewed do not establish a universal duration. State the review period and check-in dates in the plan, following employer policy and HR guidance for the circumstances.

Should an employee sign a PIP?

There is no universal signature rule in the sources reviewed. Follow established policy and specify how comments or acknowledgment will be recorded. Ask HR about applicable workplace requirements.

Can a PIP include training or coaching?

It can include support actions such as coaching, training, tools or clearer instructions when those actions are relevant to the stated objective. Record the action, owner, due date and completion evidence separately from the performance observation.

What if some goals are met but others are not?

Record each measure against its stated criteria, include relevant evidence and employee input, and have the designated decision owner review next steps under policy. The template cannot determine the outcome.

Can a PIP determine whether termination is appropriate?

No. A template organizes information; it does not determine legal sufficiency or an employment consequence. Use the employer’s established process and involve HR or counsel where appropriate.

Final selection checklist

  • Use a dedicated PIP document for a formal employee-specific improvement process.
  • Keep raw observations, citations, support actions and meeting records separate.
  • Use corrective-action analysis for process failures and development plans for proactive growth.
  • Use Forms and Automations for intake and reminders only, with human review gates.
  • Use database-enforced uniqueness or transactional upsert when concurrent workers create records.
  • Test permissions, retention, API field behavior and automation timing before rollout.
